To manage symptoms in the home environment and avoid the unnecessary risks of hospitalization, we would like to inform you of the available treatments available to our patients through hospice care in the home or facility. The necessity of these interventions is determined at the discretion of the hospice interdisciplinary team including the patient and/or the family, RN Case Manager, and the physician.
